Duties, Accomplishments and Related Skills:
Functioned as a social work intern under the direction of the homeless shelter manager of Lowndes Associated Ministries to People, (LAMP). Assisted and case managed clients from a wide variety of various socio-economic, cultural, ethnic, educational, and other diversified backgrounds. Clients included both families and individuals who were homeless or at imminent risk of homelessness. Duties included conducting assessments, developing individualized case plans, linking individuals with community-based housing and service resources, ensuring technology based resources were available and accessed (job searches, community information, etc.), helping manage finances (security deposits, rent, savings, etc.) as well as provided case management services in conjunction with community partners. Ability to work with community organizations and develop joint ventures. Organized Homeless Connect Campaign for surrounding area homeless population by contacting stakeholders and area businesses and asking for participation in community event by donating items, providing services, as well as distributing literature to homeless population about the various services available. Maintained precise and accurate documentation of case management services, including client files and entries into Pathways Compass program databases. Knowledge of personal financial management practices and techniques. Assisted clients with establishing and meeting housing goals and identifying appropriate steps towards achieving goals such as budgeting and practicing financial skills, modifying goals as needed, and evaluation of progress. Skill in conducting interviews to address concerns, needs, establishing plans/goals, and referral services as needed. Ability to meet the client where they are by conducting motivational interviews and completing a psychosocial assessment and acting upon critical needs appropriately and immediately. Assessments were independently conducted and used in identifying the patient's problems, strengths, weaknesses, coping skills, and psychosocial functioning.
Case management of clients involved developing a comprehensive client-driven case plan with both short-term and long-term goals, creating a timeline and measures for each goal, monitoring progress towards goals in regularly scheduled weekly sessions, evaluating and adjusting case plans as needed and providing written warnings with consequences if satisfactory progress was not being met. Case sessions also included empowering clients to become involved in their own planning and goal setting by referring clients to appropriate community resources to assist with meeting goals and returning clients to permanent housing as soon as possible. Referrals to various community services such as SNAP, Social Security, DMV,WIC and mental health were also completed by identifying high risk patients as well as providing psychosocial treatment during sessions for those clients who had a diagnoses of a mental health disorder as well assessing clients for suicidal ideations and drug/alcohol problems. Motivational interviewing and Cognitive Behavioral Therapy techniques were also used with group members in individual sessions. Other case management duties included conducting a class on risk and protective factors related to personal safety and how alcohol/drugs can impact decisions. Group was composed of women participants who also included women veterans that had a history of sexual trauma. Research for class consisted of identifying DoD Principles of Prevention and becoming familiar with CDC and DoD regulations relating to Sexual Assault Prevention Strategy. Profession knowledge of laws, regulations, and policies relating to active duty/civilians was obtained through research and education as well as an in-depth review of Sexual Assault Prevention Strategy.Ability to provide services, education, and training to personnel, and community agencies relating to primary prevention programs and activities due to social work experience in facilitating classes, education and training to various populations as well as working within a team environment. Experience in data collection methods by conducting assessments and surveys in identifying various populations relating to goals/needs of agency.
